We can retrieve the following types of documents from Cuba:

• Baptismal Records
• Birth Certificates
• Citizenship Certificates

1. Please enter as much information as known in the form below about the certificate you are requesting.
2. Please give us your first name and last name and contact information. We need your telephone and email address so we can contact you to confirm your request, ask questions, etc.
3. We need the full name of the person whose name is on the certificate at the time of birth. Include middle name and mother's maiden name.
4. The Date of Event is the date of the birth, death, marriage etc. that will be on the certificate. It does not mean today's date.
5. Please provide the municipality or neighborhood if known.
6. Some people have the book and page number of their certificate. Please provide this if you have it.
7. Parent's names required for birth certificates.
8. Spouse's name is required for marriage certificate.
9. If you only need your certificate for personal use, please check the For Personal Use ONLY box. Otherwise, all certificates will be legalized for official use outside of Cuba. If you are presenting the certificate to any government agency, you need a legalized certificate.
10. Use the comment area for any additional information like second phone number or email, historical information and other notes. The more information we have, the better chance we can locate your certificate and do it in a timely manner.
